Both are GLP-1-based injectable medications for weight management. Tirzepatide activates both GIP and GLP-1 receptors (dual agonist); semaglutide activates GLP-1 only. No. WISH uses asynchronous (store-and-forward) telemedicine, which is authorized under Washington, Oregon, and Montana telehealth law. You submit your health information via a secure online form and a physician reviews it without a real-time appointment.

Compounded medications are prepared by licensed 503A pharmacies under physician oversight and are regulated by state boards of pharmacy. They are not FDA-approved and have not undergone the same clinical evaluation as branded medications. All WISH prescriptions use Hallandale, a licensed 503A pharmacy with established quality controls. Your physician only prescribes when clinically appropriate after reviewing your individual intake.
If our physician determines the program isn’t appropriate for you, your card is never charged. You may receive a recommendation to schedule a traditional telehealth consultation instead.
No. WISH Weight Management is a cash-pay program. We do not bill insurance and do not provide superbills. Pricing shown is all-in: medication, physician review, injection supplies, and 2-day cold-chain shipping.
Yes. If you are currently taking tirzepatide or semaglutide through another provider, you can transfer to WISH and continue at your current dose — no restart required. As a transfer patient, you get 10% off standard pricing automatically at Levels 2–5 (Level 1 uses the same first-order price as new patients) — no code needed. Use the standard new-patient intake form for your medication and note your current dose in the form.
Submit a refill request at wishcares.org/weight-management/refill/ about 7–10 days before you run out. A physician reviews within 3–5 business days. If approved, the card on file is charged automatically for the approved dose — no call needed. If it’s not approved, we’ll reach out to you directly.
Not as a disqualifier — if anything, it’s relevant context your physician wants to know. Tirzepatide in particular has trial data (SURMOUNT-OSA, 2024) specifically in patients with obesity and moderate-to-severe sleep apnea, showing meaningful improvement in apnea severity alongside weight loss.
