How It Works

From Intake to Your Door in Days

01
Complete Your Intake

Secure online form covering your health history, eligibility, and goals. About 10–15 minutes. No video call or in-person visit required at any point.

02
Physician Review

A board-certified WISH physician reviews your intake within 1–3 business days. You receive an email with the outcome — approval or a note if more information is needed.

03
Prescription & Payment

If approved, your saved card is charged and a prescription is sent to the 503A pharmacy. If not approved, your card is never charged — no refund needed.

04
2-Day Delivery

Medication ships via temperature-controlled cold-chain courier and arrives within 2 business days of processing. Every order includes a complete injection kit.

Your relationship with a WISH physician doesn’t end at delivery — every refill request goes through the same individual review, so your dose can be adjusted as you respond rather than locked in from day one.

Sleep & Metabolic Expertise

Obesity and obstructive sleep apnea share the same root metabolic problem — which is why in the 2024 SURMOUNT-OSA trial (NEJM), the same medication class prescribed here cut sleep apnea severity by up to nearly 60% alongside weight loss. Dr. Patel is board-certified in both internal medicine and sleep medicine, and built this program specifically for that overlap. Learn more about sleep apnea →

📄

No Charge Unless Approved

Your card is only charged after a physician approves your prescription — if it isn’t approved, you’re simply never charged, so there’s no refund to process. No risk to start an intake — physician review is part of the process, not a formality.

Dr. Patel personally reviews every patient intake and refill request for the WISH Weight Management program. His training in internal medicine and sleep medicine gives him a clinically grounded perspective on metabolic health — weight and obstructive sleep apnea share overlapping mechanisms well-documented in trials like SURMOUNT-OSA, and this program is built with that connection in mind. No intake is rubber-stamped. Every approval is a clinical decision.

FAQ

Frequently Asked Questions

What is the difference between tirzepatide and semaglutide?

Both are GLP-1-based injectable medications for weight management. Tirzepatide activates both GIP and GLP-1 receptors (dual agonist); semaglutide activates GLP-1 only. Clinical trials show tirzepatide typically produces greater average weight loss (15–21% vs. ~15%), but semaglutide has a longer published safety track record. Semaglutide starts at a lower price. Your physician reviews your intake for both options; you select which to start. Tirzepatide details →   Semaglutide details →

Do I need a video call or in-person appointment?

No. WISH uses asynchronous (store-and-forward) telemedicine, which is authorized under Washington, Oregon, and Montana telehealth law. You submit your health information via a secure online form and a physician reviews it without a real-time appointment.

What states do you serve?

Washington, Oregon, and Montana. WISH physicians are licensed in all three states. We are not currently able to prescribe to patients in other states.

Are compounded medications safe?

Compounded medications are prepared by licensed 503A pharmacies under physician oversight and are regulated by state boards of pharmacy. They are not FDA-approved and have not undergone the same clinical evaluation as branded medications. All WISH prescriptions use Hallandale, a licensed 503A pharmacy with established quality controls. Your physician only prescribes when clinically appropriate after reviewing your individual intake.

What if I’m not approved?

If our physician determines the program isn’t appropriate for you, your card is never charged. You may receive a recommendation to schedule a traditional telehealth consultation instead.

Is this covered by insurance?

No. WISH Weight Management is a cash-pay program. We do not bill insurance and do not provide superbills. Pricing shown is all-in: medication, physician review, injection supplies, and 2-day cold-chain shipping.

I’m already on a GLP-1 from another provider. Can I transfer?

Yes. If you are currently taking tirzepatide or semaglutide through another provider, you can transfer to WISH and continue at your current dose — no restart required. As a transfer patient, you get 10% off standard pricing automatically at Levels 2–5 (Level 1 uses the same first-order price as new patients) — no code needed. Use the standard new-patient intake form for your medication and note your current dose in the form.

How do refills work?

Submit a refill request at wishcares.org/weight-management/refill/ about 7–10 days before you run out. A physician reviews within 3–5 business days. If approved, the card on file is charged automatically for the approved dose — no call needed. If it’s not approved, we’ll reach out to you directly.

I have sleep apnea. Does that affect whether I’m a candidate, or which medication I should use?

Not as a disqualifier — if anything, it’s relevant context your physician wants to know. Tirzepatide in particular has trial data (SURMOUNT-OSA, 2024) specifically in patients with obesity and moderate-to-severe sleep apnea, showing meaningful improvement in apnea severity alongside weight loss. Mention your sleep apnea status and any CPAP use in your intake so your physician can factor it into your evaluation and medication choice. More on sleep apnea →
