Who Qualifies for the WISH Tirzepatide Program

You likely qualify if…

  • Are 18 years or older

  • Live in Washington, Oregon, or Montana

  • BMI 30 or higher, OR BMI 27–29.9 with a weight-related condition (type 2 diabetes, hypertension, sleep apnea, PCOS, high cholesterol, or joint pain)

  • No personal or first-degree family history of medullary thyroid carcinoma or MEN2

No. WISH uses asynchronous (store-and-forward) telemedicine, which means our physician reviews your intake form without a real-time appointment. This is authorized under Washington, Oregon, and Montana telehealth law. You submit your intake; we review and respond by email.

How quickly will I see results?

Most patients notice appetite reduction within the first few weeks of starting. Meaningful weight loss typically becomes apparent by months 2–3. What if my prescription isn’t approved?

Can I stay at the same dose, or do I have to escalate?

You are never required to escalate. Dose changes are discussed with your physician at each refill and are based on tolerability and your progress — not a fixed escalation schedule. Many patients reach their goal dose and maintain it comfortably for months.

Is this covered by insurance?

No. WISH Weight Management is a cash-pay telemedicine program. We do not bill insurance and do not provide superbills. The pricing shown is all-in: medication, physician review, shipping, and supplies.

What states do you serve?

Washington, Oregon, and Montana. WISH physicians are licensed in all three states. We are not currently able to prescribe in other states.

What happens if I want to stop?

Contact us at [email protected]. There is no subscription — each order requires a separate intake or refill approval. You may pause or stop at any time. Note that weight regain is common when GLP-1 medications are discontinued; your physician can discuss a tapering plan if relevant.

Is tirzepatide a short-term program or something I stay on long-term?

For most patients, long-term. Obesity is generally treated as a chronic condition, and the SURMOUNT trials followed patients for up to 72 weeks with continued benefit rather than a defined stopping point. Some patients eventually transition to a lower maintenance dose once they reach their goal; others stay at their effective dose indefinitely. Your physician revisits this with you at each refill — it’s never a one-size schedule.
